Requesting a Course Load Reduction for Medical Reasons

A course load reduction means a student will keep and eventually receive credit for at least one class (includes incompletes).

Undergraduate Students:
A request for a course load reduction for medical reasons that occurs after the end of the eighth week of a semester is approved only in exceptional circumstances such as serious illness.  If the withdrawal is approved by the dean (or the dean’s designee), the withdrawal slip is routed to the faculty member, who indicates whether the student is passing or failing at the time of the withdrawal. If the student is passing the course, the withdrawal will be recorded as a W; if the student is failing, an F will be assigned and that grade will be calculated in the student's grade point average.

Graduate Students:
For each graduate course in which a student is doing passing work (C or better in a graduate course) at the time of withdrawal, as assessed by the instructor, a WP will be received; for any course in which the instructor determines that the student is not doing passing work, a WF will be received. If the instructor does not make an assessment of whether the student is passing or failing, a WP will be received. Transcript entries of WP and WF are not included in the computation of the graduate GPA. Transcript entries made in connection with withdrawals from undergraduate courses will be W or F in accordance with the undergraduate grading system; the withdrawal procedures and deadlines, however, will be those applicable to graduate-level students and courses.

Any students requesting a course load reduction for medical reasons are required to:

  1. Contact your Academic Advising Dean's Office to discuss your request
    for a course load reduction due to medical reasons. (Based on the UHS recommendation and other pertinent documentation, the Dean's Office makes the final decision whether the course load reduction will be granted). A formal letter of this request is required and must include your name, Z ID, and signature.

  2. Contact the University Health Service Director's Office, UHS 422,
    (815-753-1316 or 815-753-1314) for information about the medical
    review process and forms to be completed as part of this process.

    Forms for Medical Course Load Reduction

    • Student must arrange for submission of pertinent medical information from his/her medical provider(s) to the UHS Administrative Physician for review. This information must document the specific medical circumstances necessitating the withdrawal from the university.
    • Student must also sign a UHS application an authorization form to confirm his/her request for a course load reduction for medical reasons and to authorize the UHS to verify a sufficient medical condition(s) that warrants a course load reduction for medical reasons.
  3. Contact your professor(s) to inform them of your intention to request a course load reduction for medical reasons.
