Requesting a University Withdrawal for Medical Reasons

A university withdrawal for medical reasons means you are leaving the University for the current semester and will receive no academic credit for the semester, including no incomplete.

Undergraduate Students:
A request to withdraw from the university for medical reasons that occurs after the end of the eighth week of a semester is approved only in exceptional circumstances such as serious illness. If the withdrawal is approved by the dean (or dean’s designee), the withdrawal slip is routed to the faculty member, who indicates whether the student is passing or failing at the time of the withdrawal. If the student is passing the course, the withdrawal will be recorded as a W; if the student is failing, an F will be assigned and that grade will be calculated in the student's grade point average.

Graduate Students:
For each graduate course in which a student is doing passing work (C or better in a graduate course) at the time of withdrawal, as assessed by the instructor, a WP will be received; for any course in which the instructor determines that the student is not doing passing work, a WF will be received. If the instructor does not make an assessment of whether the student is passing or failing, a WP will be received. Transcript entries of WP and WF are not included in the computation of the graduate GPA. Transcript entries made in connection with withdrawals from undergraduate courses will be W or F in accordance with the undergraduate grading system; the withdrawal procedures and deadlines, however, will be those applicable to graduate-level students and courses.

Any students requesting a withdrawal from the University for medical reasons and requesting refund of payments made for tuition and fees (and room and board if living in university residence halls) are required to:

  1. Contact your Academic Advising Dean's Office to discuss your request for
    a university withdrawal for medical reasons. A formal letter of this request is required and must include your name, Z ID, and signature.

  2. Contact the University Health Service Director's Office, UHS 422,
    (815-753-1316 or 815-753-1314) for information about the medical review process and forms to be completed as part of this process.

    Forms for Withdrawal for Medical Reasons

    a. Student must arrange for submission of pertinent medical information
    from his/her medical provider(s) to the UHS Administrative Physician for review. This information must document the specific medical circumstances necessitating the withdrawal from the university.

    b. Student must also sign a UHS application an authorization form to confirm his/her request for a withdrawal for medical reasons and to authorize the
    UHS to verify a sufficient medical basis for the withdrawal for medical reasons.

  3. Write to the Office of Student Affairs, Altgeld Hall 208, indicating that they are withdrawing from the university for medical reasons and are requesting
    a refund of payments made for tuition and fees (and room and board if living
    in university residence halls).

  4. Check out from university residence halls within twenty-four hours of withdrawal from the university. (Check-out includes removing personal belongings, signing the room condition report and returning keys and
    must be done with a residence hall staff member.)

International Students:

International students are required to comply with SEVIS Rules and Regulations by providing documentation to the International Student and Faculty Office for a medical course load reduction or medical withdrawal request in any semester. Prior to requesting a withdrawal through their college advisement office, all international-status students must contact the International Student and Faculty Office at (815) 753-1346. This option is limited to a total of 12 months under current Immigration Law.
